There is a nonprofit organization based out of Brooklyn that offers a variety of online services, including 4 live chat rooms that address topics ranging from losing a pet to anticipatory bereavement. You can check it out at:

Closer to home (Illinois), I have found a service that's out of Orland Park. It is run by a husband and wife team called, 'Cubby's Compassionate Pet Service'. They provide at-home euthanasia with the help of licensed vets who accompany them on house calls. They can be checked out at:
www.CubbyCares.com
When we had to put Max to sleep back in Aug. 05 I hated taking him to the vet's office. It was cold, smelled like a hospital, and made us all so nervous.
The next time I have to go through this I want it to be at home with us in a peaceful setting.
